Naturopathic Medical Visits
Your first visit will consist of a detailed history and physical exam. Please allow for one and a half hours for an initial comprehensive visit. An acute visit, reserved for self-limiting conditions, requires 45 minutes. Please allow 1 hour for the initial pediatric visit. Prior to your visit you will be asked to fill out a detailed health history form. We request that you have this form filled out prior to your scheduled appointment. Adult New Patient Expanded: $200
Adult New Patient Minimum: $170
Pediatric Initial Visit (age 12 and under): $150

We can e-mail, fax, or send the form to you if needed. Please arrive 15 minutes early for your first appointment.
Please bring copies of all recent laboratory work and reports along with all supplements and medications you are taking.
We make every attempt to be on time for your appointment and request your cooperation in helping maintain timeliness.
Scheduling:
To schedule an appointment please call 909.796.6407.
Appointments: Office hours are Mondays 9 AM to 5 PM, Wednesdays 9 AM to 5 PM, & Fridays 9 AM to 6 PM. Our office is closed on Tuesdays & Thurdays.
Cancellations: We have a 24 hour cancellation policy. If you are unable to keep your scheduled appointment kindly call in advance of 24 hours to reschedule. Late cancellations and missed appointments will be billed at $50. We understand that emergencies occur and therefore we will make exceptions in such circumstances.
Payment: Payment is expected at the time of service. We accept cash, personal checks, Visa, Mastercard, American Express and Discover.
Insurance: Altlhough we are not contracted with insurance carriers, some PPOs provide reimbursement to their subscribers for visits to Naturopathic Doctors. You will be provided with a superbill to submit to your carrier. We encourage you to seek reimbursement, especially if you have a PPO. We do not submit bills to insurance companies directly.
